We currently have an opportunity for one (1) Social Service Worker to join our Outpatient team in Toronto, ON, on a full-time, permanent basis (OPSEU bargaining unit)
Position: Social Service Worker
Reports to: Director, Outpatient Services
Hours: 37.5 hours/week. Primarily Monday to Friday, with weekend/evening work as scheduled. This is a fully on-site position. Candidates must be available to work in person at Casey House during all scheduled work hours.
Status: Full-time permanent, (OPSEU Bargaining Unit)
Compensation: $34.34 to $42.78 per hour (7 step salary progression grid) + participation in the full-time benefits plan
POSITION SUMMARY:
The Social Service Worker is an integral member of an interprofessional team, responsible for client intake, assessment, and the provision of individual and group programming within the Outpatient Program. This role requires expertise in system navigation and emphasizes a strengths-based, client-centered approach, recognizing and amplifying individuals' inherent resilience and capabilities. It acts as a resource for the Outpatient Program team on issues affecting clients living with or at risk of HIV and those experiencing complex health and mental health and substance use challenges.
In alignment with Casey House’s mission and values, the Social Services Worker is expected to provide compassionate, dignified, and judgment-free care. They must integrate a trauma-informed, harm reduction, and client-centered approach, ensuring that services are inclusive and responsive to the diverse experiences of those accessing care. The role requires a commitment to health equity, advocacy, and fostering a safe, affirming environment for all clients
RESPONSIBILITIES
Client Intake and Assessment
- Respond to and process intake referrals for Casey House services.
- Engage clients to gather comprehensive information about their issues, needs, and goals.
- Identify priority client issues requiring immediate assessment and/or intervention.
- Retrieve collateral information from external providers and referrers.
- Present client referrals to the interprofessional team for review.
- Work closely within an interdisciplinary team to provide holistic care.
- Accurately refers clients to matched members of the interprofessional team.
- Maintain timely and accurate documentation within the electronic medical records (EMR) system.
Case Management and Client Support
- Conduct assessments and develop individualized client service and care plans.
- Provide short-term case management services tailored to client needs.
- Assist clients in accessing appropriate healthcare, housing, addiction, and mental health services.
- Provide de-escalation, conflict resolution, and crisis intervention as needed.
- Facilitate referrals to community-based HIV, addiction, and mental health services.
- Accompany clients to medical and social service appointments when necessary.
- Participate in interdisciplinary team meetings and case conferences.
Program Development and Community Engagement
- Develop and implement outreach activities to engage individuals at risk.
- Design and facilitate group programming relevant to clients' needs.
- Build and maintain relationships with community organizations.
- Advocate systemic changes to improve service accessibility.
- Participate in community-based networks, education initiatives, and advocacy efforts.
- Contribute to program development and quality improvement initiatives.
Documentation and Reporting
- Maintain case notes, statistics, centralized files, and electronic database records.
- Assist in preparing statistical and written reports.
- Ensure compliance with Casey House policies, procedures, ethical guidelines, and professional standards.
QUALIFICATIONS
Education and Experience
- Diploma in Social Service Work or an undergraduate degree in Social Work.
- Current registration in good standing with the Ontario College of Social Workers and Social Service Workers.
- Experience working with populations living with or at risk of HIV, along with an understanding of the social determinants of health that affect these communities, such as houselessness, food insecurity, and unemployment.
- A minimum of 3 years of experience working with clients who have complex medical conditions, mental health challenges, and substance use concerns, utilizing a strengths-based approach.
- Experience working diverse and historically marginalized populations, including racialized, Indigenous, or 2SLGBTQIA+_communities
- Experience providing case management and advocacy in community-based settings, prioritizing client empowerment.
- Excellent communication, advocacy and interpersonal skills
Technical Skills
- Proficiency in using EMR systems and case documentation tools.
- Knowledge of community resources, harm reduction principles, and trauma-informed care.
- Strong assessment, service planning, and client advocacy skills.
Additional Skills and Competencies
- Demonstrated commitment to harm reduction, health promotion, and social justice.
- Experience in facilitating individual and group interventions that build on client strengths.
- Ability to work effectively in a fast-paced, interprofessional environment.
- Strong communication, interpersonal, and conflict resolution skills centered on client capabilities.
- Cultural competency and experience working with marginalized populations, recognizing their strengths and community ties.
