Position: Social Service Worker

Reports to: Director of Inpatient Services

Location of work: Casey House Inpatient Unit (On-site)  

Hours: 37.5 hours/week with flexibility for weekend, evening work to meet client needs. One weekend shift is required every 6 weeks. 

Status: Full-time permanent, (OPSEU Bargaining Unit)

Compensation: $34.34 to $42.78 per hour (7 step salary progression grid)

 

POSITION SUMMARY:

The Social Service Worker (SSW) is responsible for providing information, education, and system access and navigation for inpatient clients. This position collaborates with the Registered Social Worker to address the needs of clients and their networks of support. Additionally, the SSW works directly with clients to develop essential life skills that foster independence and enhance their quality of life.

  

RESPONSIBILITIES

  • Works collaboratively with the Inpatient Social Worker to provide focused support and coordination of services, ensuring client psycho-social needs are met.

  • Providing coaching or educational sessions to help clients develop life skills like budgeting, time management, communication, and decision-making to support their independence and well-being.

  • Initiates referrals to community-based services matched to client needs;
    Initiates client obtaining identification, health card, and any documentation to support their access to health and community based services;

  • Identifies SSW and client directed goals and documents them to plan of care

  • Ensures consents for community providers, informal supports, and significant others are documented;

  • Participates in weekly interdisciplinary team meetings for the Inpatient program, as well as weekly social work meetings and care plan rounds across departments, to enhance communication and collaboration between teams.
  • Responsible for organizing discharge planning meetings and coordinating related activities, ensuring the involvement of the inter-professional team and appropriate community resources including our outpatient program.

  • Collaborates closely with the interdisciplinary team, including peers, to provide comprehensive support for clients during and after discharge.

  • Provides harm reduction education and support including safety planning around substance use
  • Communicates effectively with family/significant others/community supports/volunteers & external care providers;
  • Networks with community organizations to develop meaningful partnerships to meet the needs of the Casey House client population; 

  • Maintains professional development through seminars, workshops, and professional affiliations as approved by the Director of Inpatient Services.

 

QUALIFICATIONS 

  • Completion of a Social Service Worker diploma or degree program from a recognized institution, with coursework and practical experience in client-centered support, life skills training, and case management.
  • Registered with the Ontario College of Social Workers and Social Service Workers;
  • Clinical experience in system navigation, HIV/AIDS, working with marginalized/vulnerable populations, harm reduction, housing, substance use and/or mental health;
  • Strong verbal and written communication skills; excellent computer skills;
  • Hospital and/or health care experience preferred.
