About the Opportunity
We currently have an opportunity for one (1) Social Worker to join our Inpatient Program in Toronto, ON, on a full-time, permanent basis.
Hours of work: 37.5 hours/week (Monday to Fridays, with occasional weekend coverage)
The Inpatient Social Worker at Casey House will be an integral member of the interdisciplinary healthcare team, providing comprehensive psychosocial support to clients within the Inpatient Program. This role involves conducting client assessments, developing individualized care plans, delivering counseling services, and facilitating access to resources that promote client wellness. The Social Worker will address complex issues such as HIV/AIDS, mental health challenges, substance use, and end-of-life care, ensuring a compassionate and client-centered approach. Additionally, they will act as an advocate for clients, fostering partnerships and collaborating with community organizations to support clients' holistic needs. The ideal candidate will possess strong interpersonal skills, clinical expertise, and a deep commitment to advancing equitable and inclusive care for the LGBTQ+ community and those affected by HIV/AIDS. They should have a strong understanding of palliative and end-of-life care, Medical Assistance in Dying (MAID), harm reduction principles, and trauma-informed care, ensuring that client support is both compassionate and aligned with best practices.
Your responsibilities will entail, but are not restricted to:
• Conduct thorough psychosocial assessments for clients, particularly those facing palliative or end-of-life situations.
• Collaborate with clients and the interdisciplinary care team to ensure care plans are holistic and client-centered, using harm reduction strategies and trauma-informed approaches.
• Participate in regular case conferences and interdisciplinary rounds to discuss client progress and adapt care plans as needed.
• Integrate the clients’ cultural and spiritual beliefs of the client and their family into their care.
• Support clients in managing the emotional and psychological challenges associated with HIV/AIDS, substance use, trauma and end-of-life decisions.
• Develop individualized crisis safety plans for clients identified as high-risk, with input from interdisciplinary team members.
• Empower clients to advocate for their own needs by teaching them how to navigate healthcare and social service systems effectively
• Educate clients and their families about palliative care options, including MAID.
• Support families in understanding their loved one’s condition, discussing anticipatory grief, and preparing for the end-of-life process.
• Offer compassionate support and guidance for clients and families considering or engaging in MAID, ensuring informed and ethical decision-making while navigating the complex emotional and practical decisions related to end-of-life care.
• Assist clients in documenting their healthcare wishes, such as advanced directives or power of attorney for personal care.
• Offer grief counseling to families during the client’s palliative phase and after their passing, helping them cope with loss and find support systems.
• Work with chaplains, spiritual care providers, or cultural leaders to support clients’ spiritual needs at the end of life.
• Lead discharge planning to ensure seamless transition from inpatient care to outpatient services or other community services, including follow-up coordination
• Respond to urgent client needs relating to housing, loss of income, for food insecurity by coordinating with community resources.
• Assist clients in transitioning between care settings, such as from inpatient to outpatient, ensuring continuity of support.
About You
The following qualifications will ensure your success in this role:
Education and Experience:
• Master’s degree in social work (MSW), with current registration with the Ontario College of Social Workers and Social Service Workers
• Certifications in Palliative Care (LEAP) and grief counseling are considered an asset
• First AID/CPR Certification
• Minimum 5+ years experience working within a harm reduction framework with clients using substances, and with marginalized populations such as LGBTQ+ individuals, clients with HIV/AIDS or at risk of HIV, people experiencing homelessness, or other at-risk groups providing mental health and substance use counseling with proven ability to address concurrent disorders
Additional Skills and Competencies:
• Knowledge of social determinants of health and how they impact vulnerable populations.
• thorough understanding of end-of-life issues and competency applying palliative care principles.
• Comprehensive knowledge of mental health and addiction frameworks, including harm reduction, motivational interviewing, CBT and DBT.
• Familiarity with trauma-informed approaches and the ability to integrate these into practice.
• Proficiency in current assessment, psychotherapy and psychosocial counselling modalities and techniques for working with individuals, couples, families and communities.
• Demonstrated commitment to and knowledge of harm reduction, anti-oppressive frameworks of practice.
• Extensive knowledge of the issues arising from disabilities, deteriorating mental/physical health and of the conditions which contribute to social isolation, discrimination and racism.
• Comprehensive understanding of community-based services and supports which address issues of poverty, gender equity, homelessness, marginalization, mental health, substance use and palliative care.
• Demonstrate experience in case management and care planning for complex needs
• Strong crisis intervention and counseling skills, particularly in end-of-life and grief scenarios.
• Excellent verbal and written communication skills for working with patients, families, and healthcare professionals within multidisciplinary teams.
• Strong advocacy skills to support clients' rights and needs within healthcare and community settings, with a proven commitment to providing equitable, inclusive, and culturally competent care.
• Proven ability to manage sensitive and confidential information with discretion and professionalism.
• Proven commitment to professional development and continued learning through workshops and conferences.About the Benefits
