About the Opportunity

We currently have an exciting opportunity for a Clinical Pharmacist to join us in Toronto, ON on a permanent full-time basis. This position is part of the OPSEU bargaining unit. 

As the Casey House Pharmacist, you are an active member of the inter-professional medical team and directly responsible for health promotion and maintenance of prescription medication, including tailoring therapy to specific populations (i.e. women, immigrant or indigenous populations), clients with co-morbidities and/or co-infections (i.e. Hep B and C), and monitoring the response to therapy and managing adverse drug reactions. 

More specifically, your responsibilities in this role will entail, but are not restricted to:

  • Support medication reconciliation for inpatient admissions and medication review for patients with prolonged lengths of stay
  • Provide medication reconciliation/consultation for outpatient clients as per the medication reconciliation policy
  • Consult on inpatients for initiation of new therapies or changes to current regimens
  • Client and group education re: medication interactions, polypharmacy, drug interactions including with illicit substances, adverse and side effect management, concomitant complementary or alternative medications, health promotion with chronic illness, co-morbidities, co-infections, and medication safety
  • Supporting transitions of care from inpatient to outpatient or discharge to community, with a focus on quality improvement and safety strategies
  • Pharmacy and medication management: inventory control, regular auditing (as per Accreditation Canada standards), cost-saving strategies, electronic healthcare record (currently BCare) pharmacy linkage, and external pharmacy relationship management
  • Attend and contribute to interdisciplinary inpatient care rounds
  • Attend and contribute to outpatient team rounds, oversee injectable antiretroviral and depot clinics
  • Lead and organize Casey House’s Drugs and Therapeutics Committee including reviewing of medication incidents and improvement activities
  • Develop, implement, and update policies related to medication management
  • Act as a leader for Medication Management for Accreditation Canada Standards and point person for regular Ontario College of Pharmacy inspections
  • Be involved in the implementation and management of quality improvement projects related to medication management

About You

In order to qualify for this role, you must be fully licensed by the Ontario College of Pharmacists, with no restrictions on practice. 

The following qualifications will ensure your success in this role:

  • Training as an HIV Pharmacist, with expertise in initiating and changing anti-retroviral, managing drug-drug interactions, and optimizing drug treatment for opportunistic infections
  • Experience with counselling clients on drug interactions, polypharmacy, interactions with illicit substances
  • Experience in preventing and monitoring for resistance to ARVs, managing interactions with concomitant, complementary, or alternative medications
